Study Hall Form

Teenworks provides a safe, supervised study hall, if a student needs to have a supervised place to stay while waiting for the next class, waiting for transportation, or a place to study. This is held in the Lunch room or the quiet study hall is held in the Resource Center. You must fill out the form and then you will be allowed to stay in the study Hall.
  • Put name of student.
  • Please enter a value between 0 and 20.
    check time period(s) you will be in study hall.
  • choose the number of time periods you will be in study hall.
  • Price: $32.00
    $32.00/semester for each study hall that the student uses.
